Welcome New Patients
We welcome new patients! We love it when you refer your friends and family. That’s how our practice grows. When you call for your first appointment, Pat will mail you a health history form or you can download the form from this website. Fill out the forms at your leisure and bring them to your appointment. If you are coming as an emergency patient, we can fill out the necessary forms at the time of your visit.
Your first exam will be a thorough and extensive evaluation of your dental needs. If you do not have recent x-rays, we suggest that you have them taken at this visit. X-rays, though kept to a minimum, are necessary for diagnosis. If treatment is necessary, Dr. Sims will go over all the options available and discuss cost.
Financial Matters
We accept all forms of payment at the time of service. Claims are filed immediately on your behalf with all carriers and an in-house, affordable plan is available.
 Dr. Sims accepts Care Credit. Click here for more info. Like a credit card, but better CareCredit works like a credit card, but is exclusive for healthcare services. CareCredit offers low monthly payment plans for healthcare procedures not commonly covered by insurance, including dental treatment.  We accept most credit cards. We are a fee-for-service practice but we are happy to file your dental insurance and pre-determine your coverage. Other payment options are available for larger cases. We assume you don’t like surprises.
